ASEQ (Québec Student Health Alliance)

The AÉCSP provides (through our partner ASEQ) two additional types of insurance on top of RAMQ or ASHEE: the Health plan and Dental & Legal plan. Payment is processed in your tuition fees

These two plans are optional and exist in a basic and subsidized version.

As a graduate student at Polytechnique Montréal, you are signed up by default for the subsidized Health + Dental & legal plan.

To opt-out, an email will be sent out to you in the beginning of the semester with necessary steps.

What does the health insurance cover?
- physical therapy (part of the cost)
- contraception
- antidepressants
- psychologist (part of the cost)
- Glasses, contact lens, sight examination (part of the fees)

- Yearly dental care + examination (part of the cost)
- Tooth extraction (part of the cost)
- Legal aid, legal representation

File a claim for healthcare services
You can file claims with the OMNI app by Desjardins Insurance. For more details and to download the app, check out here. You may also file a claim by mail following this procedure:
1. Fill out and sign the health insurance claim form available here.
2. Attach original receipts and relevant documents to your form.
3. Send the documents to the insurance company at the following address:

File a claim for dental services
You can file claims with the OMNI app by Desjardins Insurance. For more details and to download the app, check out here. You may also file a claim by mail following this procedure:
1. The dentist must fill out the standard dental claim form available here.
2. You must sign and fill out your part on the form. Do not forget to include your student ID number
3. Mail the claim form to the following address:

4. The check will be issued to your name and sent to your address

Where to find a dentist or eye care professional contracted with the student health network?

Visiting a contracted professionnal will let you get an extra discount on the cost of their services.

What to do if you wish to withdraw partly or completely from additional insurance?

The collective AÉCSP insurance is optional. To opt-out, remove or add any insurance (dental or health insurance) you must perform the required changes during the authorized period in the fall. More information will be sent via email in the beginning of the semester.

AMF Survey

The Autorité des Marchés Financiers (AMF) is currently conducting a consultation on student group insurance plans, in order to verify their relevance and to consider a modification of the legislation surrounding them.

The AÉCSP would therefore like to prepare a brief to give the opinion of the Polytechnique graduate community in this consultation.

The purpose of this survey is to gather your opinion on the insurance offered by the QSHA (Quebec Student Health Alliance). The data collected will be used to help us write the brief on our insurance and the need of our population for it, as part of the AMF consultation. Your participation is welcome and will help us to best represent you on this important issue!

The survey will close on October 5th.
